Effective Date of Benefits

Your benefit coverage will start on your initial eligibility date. For example, if your employer makes contributions for you for at least 130 credited hours for work performed in January, your coverage will start on March 1. If you have dependents on the date your coverage starts, their coverage will start on that same date. If you later acquire a dependent while you are eligible, coverage will start on the date the person became your dependent. Your dependent's eligibility is contingent upon your eligibility. The minimum credited hour requirement for continuing eligibility is 130 hours per eligibility month. The following table shows how eligibility months correspond to benefit months:

EMPLOYEE AND RETIREE LIFE INSURANCE

$10,000 in employee Life Insurance is provided for active eligible employees. $7,500 in Life Insurance is provided for eligible retirees.

REMINDER for all employees who have dependent children in college. You must get a signed letter from the Registrars office each semester (fall and spring) verifying that they are registered as full time students. They will not accept a copy of their schedules, it must be a letter and it must come from the Registrars office. If the Insurance Company does not have this letter at the beginning of each semester your child will be dropped from the insurance.
